To apply for membership please print out both PAC MEMBERSHIP APPLICATION FORM and MEMBER INFORMATION INTAKE form, fill them out and mail to the PAC office in Hamtramck, MI.
Fees:
- a $35.00 application fee should be included with the application for membership (Non- refundable)
- the $35.00 will be applied to the yearly dues for the first year of membership
- membership fees are due in January of each year.
