To apply for membership please print out both MEMBERSHIP APPLICATION form and MEMBER INFORMATION INTAKE form, fill them out and mail to the PAC office in Hamtramck, MI .

Fees:

• a $35.00 application fee should be included with the application for membership (Non- refundable)
  
• the $35.00 will be applied to the yearly dues for the first year of membership
  
• membership fees are due in January of each year.
